Clinical Services Pathway Coordinator (1.0 FTE PERMANENT)

The Clinical Services Pathway Coordinator provides appropriate case assignments and access to service/information to children and families who are waiting for service delivery or transitioning from service. The Clinical Services Pathway Coordinator will work collaboratively with all managers across the Clinical Services Division to ensure a seamless, integrated approach to service delivery from the point of referral. The Coordinator will also liaise with referral sources and other community service providers with respect to referral status and transitions.

Specific Responsibilities:

Responsibilities of the position may include:

Facilitate Access to Information, Services and Supports

  • Respond to inquiries from families who are waiting for service initiation and update client records based on clinical assessment/judgement
  • Review client needs and act as a point of contact for clients waiting for service initiation including providing information about appropriate resources while waiting for service
  • Respond to other health care providers and community agencies to review shared clients waiting for service initiation
  • Apply de-escalation skills and respond to concerns and complaints. Resolve conflict and escalate significant matters to the manager
  • Triage issues that require Manager input/resolution
  • Document concerns/complaints
  • Respond to routine service pathway determination questions from program staff as a first response in collaboration with the Clinical Practice Facilitator
  • Inform the Clinical Leadership Team of common themes related to service access, pathways arising from families, referral sources and staff.
  • At the direction of the manager, develop and support universal resources and supports across the division to support children and youth waiting for service initiation

Case Assignment

  • Monitor waitlists across all portfolios and provide updates to Clinical Leadership
  • Assign clients to the appropriate clinician(s) including assessing for prioritization, urgency, clinician availability and geographical need (use of satellite sites)
  • Review waitlists to inform Clinical Leadership of necessary therapy groups within the service pathways
  • Make daily expedient decisions based on policies and procedures for caseload assignment including HCCSS referrals

Support Transition Processes

  • Facilitate the transition to school process including aggregating data about clients across the division to share with school boards and community partners and supporting staff in the appropriate processes for transition to school.

Accountability and Freedom to Act:

The incumbent has latitude for discretion within practices and procedures covered by precedents but also based on their accountability and responsibility for the areas mentioned above, This position is also subject to centre policy and procedure manuals, and supervisory review.

The Clinical Services Pathway Coordinator is concerned with the effectiveness of all relationships that affect the Centre. The following are particularly significant to the position:

Position Specifications:

Education:

The minimum specifications for an incumbent will include:
  • Regulated professional from any of the disciplines of OT, PT, SLP or Social Work. Current registration with an associated College for their discipline.

Knowledge/Skills:

An incumbent will be expected to have the following knowledge and skills:

  • A sound understanding of the Clinical Services Division. Including current service pathways.
  • A minimum of 3 years of experience in their related discipline in pediatrics preferably in a multi or interdisciplinary environment
  • Sound knowledge of the roles and delivery of therapy services across Niagara Children’s Centre
  • Knowledge of privacy and other relevant legislation
  • Sufficient computer literacy to monitor and extract all waitlist reports
  • Excellent facilitation and problem-solving skills
  • Strong conflict resolution skills
  • Excellent organizational and communication skills
  • Critical reasoning
  • Clinical assessment

Other:

  • The ability to negotiate and portray a positive image of the Centre
  • A current police criminal reference check upon entry into the position
  • Nonviolent crisis intervention training.
